Resumo Objetivo: Avaliar os achados na radiografia de tórax de pacientes com diagnóstico de infecção pelo vírus influenza. Materiais e Métodos: Revisamos, retrospectivamente, os achados na radiografia de tórax de 17 casos de infecção pulmonar pelo vírus influenza (7 do sexo masculino e 10 do sexo feminino; idade média de 14 meses, variação de 2 a 89 meses). Os pacientes foram examinados entre 2012 e 2016 e o diagnóstico foi estabelecido por critérios clinicorradiológicos e detecção do vírus por reação em cadeia de polimerase. Os achados radiológicos foram caracterizados por tipo e padrão de opacidade e distribuição por zonas pulmonares. A população estudada foi dividida em dois grupos: sem suporte ventilatório e com suporte ventilatório e/ou óbito. Resultados: A anormalidade encontrada com maior frequência na radiografia de tórax foram as marcas peribroncovasculares, a maioria delas com extensão menor de 25% do pulmão, envolvimento bilateral e assimétrico. A região mais frequentemente envolvida foi o terço médio, com distribuição central e periférica e ausência de derrame pleural. Houve diferença estatisticamente significante na simetria do envolvimento pulmonar, entre os grupos, havendo preponderância de achado assimétrico (p = 0,029) no grupo que necessitou de suporte ventilatório. Conclusão: Pacientes pediátricos com infecção pelo H1N1 apresentam como alterações principais na radiografia do tórax inicial marcas peribroncovasculares, opacidades alveolares inespecíficas e consolidações. Embora o diagnóstico definitivo não possa ser feito com base em imagens características isoladas, uma combinação dos achados clínicos e radiográficos pode melhorar substancialmente a acurácia do diagnóstico nessa doença.

Abstract Objective: To evaluate chest X-ray findings in pediatric patients diagnosed with influenza A (H1N1) virus infection. Materials and Methods: We retrospectively reviewed chest X-ray findings in 17 cases of pulmonary infection with the H1N1 virus (in 7 males and 10 females) examined between 2012 and 2016. The mean age of the patients was 14 months (range, 2-89 months). The diagnosis was established on the basis of clinical and radiographic criteria, and the virus was detected by polymerase chain reaction. The radiographic findings were categorized by type/pattern of opacity and by lung zone. The patients were divided into two groups: those not requiring ventilatory support; and those requiring ventilatory support or evolving to death. Results: The abnormality most often seen on chest X-rays was that of peribronchovascular opacities, the majority of which affected less than 25% of the lung, the involvement being bilateral and asymmetric. The lung zone most frequently involved was the middle third, with central and peripheral distribution, without pleural effusion. There was a statistically significant difference between the groups in terms of the symmetry of pulmonary involvement, asymmetric findings predominating in the group that required ventilatory support (p = 0.029). Conclusion: In pediatric patients with H1N1 virus infection, the main alterations on the initial chest X-rays are peribronchovascular opacities, nonspecific alveolar opacities, and consolidations. Although the definitive diagnosis of H1N1 virus infection cannot be made on the basis of imaging characteristics alone, using a combination of clinical and radiographic findings can substantially improve the diagnostic accuracy.

Resumo Objetivo: Melhorar a comunicação entre médicos assistentes e radiologistas, definindo quais informações deveriam ser incluídas num relatório radiológico e qual o formato preferido dos solicitantes de um hospital universitário. Materiais e Métodos: Os participantes foram convidados a escolher a opção preferida entre relatórios com diferentes formatos e níveis de detalhamento de três casos hipotéticos e questionados se características comumente encontradas em um laudo radiológico são apropriadas ou não para inclusão. Para atribuição da ordem absoluta de preferência dos diferentes relatórios, o método Kemeny-Young foi utilizado. Resultados: Noventa e nove médicos responderam os questionários (40,4% médicos residentes, 31,3% preceptores dos programas de residência e 28,3% professores da faculdade de medicina). Laudos estruturados com impressão ou comentário foram selecionados nas situações de ultrassonografia normal por 54% dos participantes, ultrassonografia alterada por 59% e tomografia computadorizada por 53%. As informações que deveriam ser incluídas, de acordo com os solicitantes, foram qualidade da imagem (92%), detalhes do cenário clínico (91%), impressão diagnóstica (89%), técnica do exame (72%), informações sobre meio de contraste (68%). Opiniões sobre seguimento e investigação radiológica ou não radiológica se mostraram importantes. Conclusão: Relatórios estruturados com impressão ou comentário de um radiologista são prepostos. Informações como qualidade de exame, meio de contraste utilizado e sugestões de acompanhamento e investigação adicional são valorizadas pelos médicos solicitantes.

Abstract Objective: To improve communication between attending physicians and radiologists by defining which information should be included in radiology reports and which reporting format is preferred by requesting physicians at a university hospital. Materials and Methods: Respondents were asked to choose among reports with different formats and levels of detail, related to three hypothetical cases, and questioned as to which characteristics commonly found in radiology reports are appropriate for inclusion. To assign the absolute order of preference of the different reports, the Kemeny-Young method was used. Results: Ninety-nine physicians completed the questionnaires (40.4% were resident physicians; 31.3% were preceptors of residency programs; and 28.3% were professors of medicine). For ultrasound with normal findings, ultrasound showing alterations, and computed tomography, respectively, 54%, 59%, and 53% of the respondents chose structured reports with an impression or comment. According to the respondents, the characteristics that should be included in the radiology report are the quality of the image, details of the clinical presentation, diagnostic impression, examination technique, and information about contrast administration, selected by 92%, 91%, 89%, 72%, and 68%, respectively. Other characteristics that were considered important were recommendations on follow-up and additional radiological or non-radiological investigation. Conclusion: Requesting physicians apparently prefer structured reports with a radiologist impression or comment. Information such as the quality of the examination, the contrast agent used, and suggestions regarding follow-up and additional investigation are valued.

Resumo Objetivo: Avaliar as características de imagem na tomografia computadorizada de pacientes admitidos com suspeita clínica de hemorragia adrenal aguda em três instituições, no período de três anos, e que tiveram o diagnóstico final confirmado. Materiais e Métodos: Análise retrospectiva da tomografia computadorizada de pacientes com suspeita de hemorragia adrenal aguda. Foram incluídos apenas os casos com confirmação diagnóstica. Foram excluídos os pacientes que apresentaram ruptura aórtica e hemorragia retroperitonial. As imagens foram analisadas por um radiologista experiente e dois residentes em radiologia. Resultados: Este estudo analisou seis casos de hemorragia adrenal unilateral, três deles na adrenal esquerda e três na adrenal direita. Todos apresentaram-se com massa arredondada ou oval, alterando a anatomia habitual da glândula. O diâmetro máximo foi 12,2 cm e os valores de atenuação variaram de 45 a 70 UH, não se observando realce nas fases contrastadas. Não foram observadas calcificações. Conclusão: As hemorragias adrenais são raras, porém, potencialmente fatais. O diagnóstico precoce é essencial, portanto, o reconhecimento das possíveis apresentações dessas lesões pelos radiologistas pode ajudar a evitar que pacientes evoluam com insuficiência da glândula e óbito.

Abstract Objective: To evaluate the computed tomography characteristics of patients admitted with clinical suspicion of acute adrenal hemorrhage at three centers over a 3-year period and in whom that diagnosis was ultimately confirmed. Materials and Methods: This was a retrospective analysis of computed tomography findings in patients with suspected acute adrenal hemorrhage. We included only those cases in which the diagnosis was confirmed. Patients with aortic rupture and retroperitoneal hemorrhage were excluded. The images were analyzed by an experienced radiologist and by two radiology residents. Results: Six cases of unilateral adrenal hemorrhage (three on the left and three on the right) were analyzed. On computed tomography, each appeared as a rounded, oval mass altering the usual anatomy of the adrenal gland. The maximum diameter was 12.2 cm, and attenuation ranged from 45 to 70 HU. There was no appreciable contrast enhancement. No calcifications were observed. Conclusion: Adrenal hemorrhage, albeit rare, is potentially fatal. Early diagnosis is essential. Therefore, recognition of the possible presentations of these lesions by radiologists may help prevent hemorrhage from progressing to adrenal insufficiency and death.
